Attention Incoming Freshman
The School Code of Illinois requires that each student entering 9th grade have a physical examination completed by a licensed physician, advanced practice nurse, or physicians assistant. The physical exam must be on the State of Illinois Certificate of Child Health Exam form and up-to-date immunizations compliant with the Illinois Department of Public Health. Your child will not be given a class schedule without this health information. A sport physical will not be accepted in place of a Certificate of Child Health Examination for school entry. New state law requires all 9th graders to have proof a dental exam as well.
Attention Incoming Seniors
Please be advised that students, entering the 12th grade, are required to receive a second dose of the meningococcal conjugate vaccine (MCV4). The immunization requirement consists of two doses for the 12th grade entrance unless the first dose was administered to a child who was 16 years or older, in which case only one dose would be required at the 12th grade entrance. Proof of this requirement must be turned in to the Nurse’s office in order for your child to receive a schedule.
